Applications are now being accepted for a Casual Nurse Practitioner to work on the Mobile Medical Support (MMS) Unit. The successful applicant will be required to work the following shifts: 0800-1600, 1000-1800, and 1200-2000, Monday to Sunday including holidays.
Reporting to the Director, Community Outreach, the Nurse Practitioner will provide interdisciplinary primary care and social supports to vulnerable individuals living in high priority communities. The Mobile Medical Support (MMS) clinic is the first of its kind regionally and has received ongoing recognition both locally and provincially for the innovative and collaborative healthcare system pathway it has demonstrated. By travelling throughout Windsor-Essex County and Pelee Island to provide healthcare directly in high priority communities, MMS has demonstrated the success of meeting clients where they are at while striving to eliminate barrier to accessing healthcare and supports. The Nurse Practitioner will lead this critical work alongside the MMS frontline team either on the mobile MMS unit or indoors at partnering agencies. They may also provide virtual support to their team members at tandem clinic locations as needed and may attend community events for healthcare provision and education. Responsibilities
- Facilitate and implement systems, which promote and maintain effective communication and collaboration with physicians and nurses in the community, other members of the health care team, and community service providers
- Lead MMS frontline team members through safe and effective clinical practices to ensure highest level of care provision to a diverse population of clients
- Collaborate with community partners and patient advocacy groups to identify needs and to develop primary care programs that are culturally appropriate (ie. Indigenous, Black Health, EIDA-R, etc.)
- Work alongside the Director, Community Outreach to ensure all program goals are met and appropriate data is collected
- Develop Primary Health Care and Health Promotion programs under the direction of the A.E.D. Patient Services for the identified community partners
- Collaborate with physicians and members of MMS in the development of Medical Directives for the delivery of client care
- Consult with Physicians according to the CNO’s document, Standards for Practice – RN (EC), 1988
Knowledge, Skills, and Abilities
- Maintain and enhance a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
- Facilitate own professional development and maintain clinical competency through formal educational programs and mentoring opportunities with consultant(s) and peers
- Identify the need for and participate in scholarly/research activities
- Coach and teach staff in the health care management of selected patient populations, both formally and informally
- Assist staff as appropriate in the development of clinical decision making and problem solving skills through case consultation, clinical demonstration, interviewing, case reviews, etc.
- Contribute to the education of nurses and other health professionals through teaching, publication and presentations
- May precept postgraduate students from various programs
- Develop and participate in quality-monitoring programs for the services and routes results to service managers
- Conduct needs assessments
- Participate in the development of programs and services geared towards community outreach
- Participate in the program evaluation process and exhibit strong personal commitment to continuous quality improvement
- Maintain appropriate records and documentation of clinical and educational activities
Qualifications/Skills Required
- Current certification of competence as an RN in Ontario
- Registered with the College of Nurses on Ontario in the Extended Class – Nurse Practitioner, with specialization in Primary Care
- Knowledge of Nurse Practitioner’s prescribing guidelines as set out by the College of Nurses of Ontario
- Maintains and enhances a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
- Responsible for advancing skills and education by:
- Seeking learning experiences and responsibilities to strengthen areas requiring development
- Assessing own performance status in relation to this position description
- Setting realistic and measurable goals for individual development
- 2 years’ experience Primary Care and Community
- Triage training and experience
- Program planning and development skills in collaboration with Community Partners
